Severe hypernatraemia (221 mEq/l), rhabdomyolysis and acute renal failure after cerebral aneurysm surgery.
Hypernatraemia is a relatively frequent electrolyte abnormality in hospitalized patients, with reported incidences ranging from 0.65 to 2.23% [1]. Normally, when the serum sodium rises, thirst develops and antidiuretic hormone (ADH) is secreted. Hypernatraemia will not occur in individuals with an intact thirst mechanism and free access to water, due to a potent osmolar stimulus to drink. Hypot...

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) not only induces uncharacteristic clinical symptoms but also increases morbidity and mortality [1, 2]. The recent introduction of vaptans acting as V2-receptor antagonists considerably improved the unconvincing results of conventional treatment consisting of fluid restriction potentially combined with sodium supplementation and/or...

18p- syndrome and hypopituitarism.
A patient is described with 18p- syndrome and hypopituitarism. This is the first patient with this syndrome who has been shown to benefit from growth hormone therapy. Patients with this syndrome who have growth deficiency should be considered for evaluation for hypopituitarism, if the quality of their lives would improve with an increase in stature.

Two half-brothers with short stature secondary to growth hormone deficiency and a family history implicating X-linked transmission were studied extensively for other endocrine abnormalities. The proband had a normal physical examination, except for small stature and small external genitalia. ACTH and TSH release were normal. LH and FSH responses during an i.v. GnRH test were severely blunted. H...
